We are building the most complete picture ever assembled of who delivers HIV services across our region, where, and for whom.

Funding decisions, referral pathways, and the next Needs Assessment will all be built on this data. Agencies that do not respond are invisible in that process. Agencies that do respond are listed, mapped, and counted.

Right now, no one has the full picture.

Services are delivered across dozens of agencies, hospitals, health departments, faith-based organizations, pharmacies, housing providers, and community-based organizations. Much of that work is invisible at the system level. Clients fall through gaps that no one has mapped, because no one has ever assembled the map.

This survey changes that. It produces four things.

OutputA living provider directorySearchable by service, population, county, and language.
OutputA geographic coverage mapWhere services exist, and just as importantly, where they do not.
OutputA referral network mapWho sends to whom, and where closed-loop referrals break down.
OutputThe next Needs AssessmentYour answers on gaps, workforce, and barriers become primary source data.

How your information will be used

  • Aggregated for planning, mapping, and needs assessment
  • Shared with the regional planning body and participating partner agencies
  • Published in the provider directory only with your explicit consent, which you control in the final step
  • Never used for monitoring, audit, contract compliance, or enforcement
  • Contact names and emails are used to follow up on your responses and are not published

You will move faster if you have these nearby. Nothing here is required, and best estimates are perfectly acceptable throughout.
